Pre-Therapy and Contact Work

Pre-Therapy is a Client-Centered treatment to prepare psychotic persons, often with learning disabilities or organic impairments, for regular psychotherapy.


Contact Work uses elements of Pre-Therapy to focus on psychological contact. Contact Work has been applied to many populations; for example persons with a pervasive developmental disorder, or dementia.


Psychological contact is the psychological function of interaction with persons but also with objects, feelings, etc. Prouty has proposed three domains of psychological contact: reality, affect, and communication. In parallel, he has differentiated between three contact functions: Reality contact is awareness of people, places, things, and events. Affective contact is awareness of distinct moods, feelings and emotions. Communicative contact is the symbolization of reality contact and affective contact to others.

Pre-Therapy is an evolution from client-centered theory and practice. The focus of Pre-Therapy is on regressed patients such as the psychotic retarded or chronic schizophrenics. It is also applicable to demented populations. The central construct is psychological contact, described as the necessary condition of psychotherapy and outlined on three levels: 1) the contact reflections–the “work” the therapist does, 2) the contact functions–the psychological process of the patient, 3) the contact behaviors– operationalized behavior for measurement. The Pre-Expressive Self, an intuitive and heuristic concept, interprets the progress from pre-expressive to expressive states of communication that are necessary for psychotherapy.

La Pre-Terapia es un método psicoterapéutico para el tratamiento de personas cuya capacidad de contacto relacional se haya disminuida, deteriorada o ausente, como es el caso de aquellas que presentan psicosis, retardo mental o daño cerebral. Su constructo principal es el Contacto Psicológico, descrito como pre-condición para establecer una relación de ayuda, el cual se delinea en tres variables: 1) Las Funciones del Contacto - el proceso del cliente, 2) Los Reflejos de Contacto – la interacción que brinda el terapeuta, y 3) Las Conductas del Contacto – los resultados observables de la aplicación del método. La Pre-Terapia se propone como una herramienta útil para devolver a las personas con un contacto psicológico deteriorado a un estado de mayor conexión y comunicación. El artículo presenta los aspectos teóricos y prácticos más esenciales de este acercamiento.

La Pre-Terapia es una teoría y metodología diseñada para desarrollar o restaurar las funciones de la realidad, afectivas y comunicativas necesarias para las acciones terapéuticas con los clientes psicóticos/retardados o esquizofrénicos crónicos. Teóricamente, expande la discusión de Rogers de que el cliente y el terapeuta necesitan estar en “contacto psicológico” como la primera condición necesaria de una relación terapéutica. Sin embargo, en contraste con la teoría clásica, la Pre-Terapia no asume su presencia con las poblaciones crónicamente regresivas.

La Pre-Terapia, en la ausencia de una definición teórica de Rogers, también provee una conceptualización del “contacto psicológico”. Específicamente, la Pre-Terapia define contacto psicológico, funciones de contacto y conductas de contacto como un sistema teórico interdependiente.

Reflejos de Contacto

Los reflejos de contacto son las técnicas por las que la Pre-Terapia facilita las funciones de contacto (realidad, afecto, y comunicación). Los reflejos situacionales facilitan el contacto con la realidad. Los reflejos faciales facilitan el contacto afectivo. Los reflejos palabra-por-palabra desarrollan el contacto comunicativo. Los reflejos corporales asisten el contacto generalizado con el “aquí y el ahora”. Los reflejos reiterativos asisten el re-contacto.

Funciones de Contacto

El contacto es conceptualizado como el darse cuenta del MUNDO, de UNO MISMO, y de los OTROS. El contacto con la realidad (MUNDO), definido como conciencia de personas, lugares, cosas y eventos. El contacto afectivo (UNO MISMO) es definido como el darse cuenta de sentimientos, estados de ánimo y emociones.

El contacto comunicativo (los OTROS) es definido como la simbolización del darse cuenta de la realidad y del afecto para otros. El contacto con la realidad, afectivo y comunicativo son las funciones psicológicas necesarias para el tratamiento.

Conductas de Contacto

Como resultado de los reflejos de contacto, el cliente podría evidenciar más contacto con la realidad, afectivo y comunicativo, por ejemplo, el cliente expresa más contacto con el MUNDO, CONSIGO MISMO, y con los OTROS; por lo tanto, haciéndose más accesible para programas o psicoterapia. Como una manifestación conductual emergente, el contacto con la realidad es la verbalización de personas, lugres, cosas y eventos. El contacto afectivo es la expresión de la emoción a través de palabras de sentimientos o concretizaciones corporales y faciales. El contacto comunicativo es la utilización del lenguaje en palabras o frases. Las conductas de contacto resultan en la operacionalización necesaria para la investigación.

Pre-Therapie: eerste hulp bij aangaan van contact

Pre-therapie heeft zich ontwikkeld uit de client-centered/experiëntiële psychotherapie. Doel is om bij patiënten met contactstoornissen, ongeacht de oorsprong ervan, contact te leggen en te consolideren. De technieken van pre-therapie worden kort besproken en aan de hand van een gevalsbeschrijving geïllustreerd.

Præ-terapi er en måde at være sammen med de dårligst fungerende patienter i psykiatrien. Terapien foregår aldeles på patienternes egne præmisser og stiller ikke nogen krav om bidrag til kontakten fra deres side.

Experience with a person with autism

The aim of this work is to explore the experience of the autor with contact and the use of the method of contact reflections within the encounter with adult persons with autism. The purpose of engaging the method of contact reflections, which comes from Pretherapy (Prouty, 1976), besides an intention to facilitate contact of a person with autism and the author herself, is also to acquaint the reader with this development of 'person-centered approach'. The study is focused on the experience of the author with the contact and engaging contact reflections during intensive personal encounters with people with autism, which is self-reflectively created in the narrative through diaries. Consequently, the experience is synthetized and analyzed through heuristic phenomenological methodology according to Moustakas (1990). In the end the author points to the main components of her experience (for example: Internal change, Contact reflections as a medium of contact, Effort for the own approach,...), that are presented in the depiction of the essence of the experience and in the creative synthesis.

Het gebruik van contactreflecties: hoe ik contact heb proberen te maken met Gert en zijn wereld

Maandagmorgen 08u15, de meeste bewoners hebben zich gewassen en aangekleed maar Gert is nog op zijn kamer …

BEGELEIDER Ben je bijna klaar Gert? We gaan zo meteen eten.

BEWONER Gert kijkt even op naar me en trekt zijn jeansbroek op.

BEGELEIDER Goed hoor Gert. Neem je schoenen maar en dan gaan we eten.

BEWONER Gert kijkt naar zijn broek en doet deze weer omlaag.

BEGELEIDER Doe je broek maar terug aan Gert. We zijn al wat laat.

BEWONER Gert kijkt niet naar me op en doet zijn broek weer omlaag en omhoog.

BEGELEIDER Kom Gert, doe eens wat verder. Doe je broek nu maar vast en doe je schoenen aan…

Contact maken en/of bevorderen bij personen met een mentale handicap is niet altijd even eenvoudig. Dit heb ik ondervonden tijdens mijn stage in een bezigheidstehuis voor volwassen personen met een mentale handicap. Het hierboven beschreven voorbeeld maakte ik toen dagelijks mee. Gert is een man van 33 jaar met een ernstig mentale handicap en een autismespectrumstoornis. Spreken deed hij nooit. Soms zei hij een enkel woord. Hij vertoonde veel dwanghandelingen, zoals zijn broek bij het aan-en uitkleden steeds een x-aantal keer omhoog en omlaag te doen.

Toen ik hoorde over het gebruik van contactreflecties, leek dit iets wat ik perfect kon toepassen op de doelgroep waarmee ik werkte.

Het gebruik van contactreflecties in een notendop

Het gebruik van contactreflecties staat stil bij de vraag hoe je contact tot stand kan brengen en/of bevorderen bij personen die contactgestoord zijn. Het gaat hier dus verder dan alleen personen met een mentale handicap, bijvoorbeeld ook personen met een psychose.

Het gebruik van contactreflecties is een bepaalde methodiek die is ingebed in een bredere context, namelijk die van pre-therapie. De eigenlijke grondlegger van pre-therapie en de hierbij horende contactreflecties, is Garry Prouty. Onder contactreflecties verstaat Prouty het gedrag en de omgeving van de cliënt dat op gevoelige wijze precies en concreet wordt benoemd. Oorspronkelijk is pre-therapie bedoeld om in een therapeutische context toe te passen. Via mijn studies als opvoeder-begeleider, wilde ik ook eens weten of het toepasbaar is in een contactmilieu, bijvoorbeeld een leefgroep.

Grondslagen pre-therapie

Prouty heeft pre-therapie ontwikkeld vanuit een voornamelijk cliëntgerichte achtergrond. Bij de klassieke cliëntgerichte psychotherapie van Carl Rogers kan therapie pas mogelijk zijn wanneer er psychologisch contact is tussen therapeut en cliënt. Onder psychologisch contact verstaat Prouty het contact krijgen met de werkelijkheid, het tot stand brengen van affectief contact en het bevorderen van het communicatieve contact. Daar dit psychologisch contact er bijna nooit is bij personen met een contactstoornis, kan er ook geen sprake zijn van een reguliere therapie, zoals cliëntgerichte gesprekstherapie. Prouty ontwikkelde een manier om toch in contact te komen met deze mensen zodat hierna de gangbare cliëntgerichte psychotherapie van Rogers wel effect kan hebben. Het voorvoegsel ‘pre’ in pretherapie slaat op een methode die voor de eigenlijke therapie komt.

De methodiek of het gebruik van contactreflecties

Situational reflection: De therapeut probeert de actuele situatie en de omgeving van de cliënt te overzien en benoemt daarbij de concrete omstandigheden of het gedrag van de cliënt dat daarmee samenhangt. “Gert zit nu in de zetel in de woonkamer en is in een tijdschrift aan het bladeren.”

Facial reflection: De therapeut observeert de cliënt en verwoordt het gevoel dat onrechtstreeks van de cliënt spreekt. De mimiek van de cliënt wordt hier als aanknopingspunt gebruikt. “Ik zie dat Gert lacht!” of “Gert fronst zijn wenkbrauwen.” of “Ik zie een traan op Gert zijn gezicht.”

Body reflection: De therapeut beschrijft of spiegelt wat het lichaam van de cliënt aan het doen is. “Gert staat gebukt boven het toilet en wrijft met een stukje toiletpapier in het toilet.”

Word for word reflection: De therapeut herhaalt of neemt de zinvolle woorden, zinsdelen of affectief geladen klanken op. Meestal reageerde Gert op mijn vragen door ja of neen te knikken. Soms gebeurde het dat hij antwoordde. Als ik bijvoorbeeld vroeg of hij appelsap of appelsiensap wilde drinken, zei hij soms “appelsien”. Als hij dit zei, was mijn reflectie: “appelsien”. Gert reageerde hier dan op door me te bevestigen en te knikken.

Reiterative reflection: Het principe van opnieuw herhalen gaat over interventies die vruchtbaar bleken te zijn, die een moment van contact tot stand brachten. Deze worden dan door de therapeut herhaald. Dit principe is een belangrijk hulpmiddel om het contact te verstevigen.

Pre-expressief gedrag

Personen die contactgestoord zijn, uiten zich vaak in een taal die geen betekenis lijkt te hebben. Wat deze personen zeggen, lijkt ‘geen realiteit’ te kennen. Vaak worden deze personen gezien als personen die ‘gestoord gedrag’ vertonen.

Volgens Prouty en anderen draagt de benaming ‘gestoord gedrag’ een beoordeling op voorhand met zich mee. Het lijkt alsof er niets meer kan veranderen, terwijl Prouty ervan uitgaat dat er wel iets kan veranderen, dat het gedrag een potentieel aan reële betekenissen in zich draagt. Hij stelt dat we de expressie in het gedrag niet zien, maar dat ze wel ergens is.

Kritische bedenkingen

De grootste valkuil van het gebruik van contactreflecties is dat het bedrieglijk eenvoudig is. Het lijkt eenvoudig, nadoen of zeggen wat je ziet gebeuren. Het is echter complexer dan dat. Je moet bewust voor bepaalde contactreflecties kiezen, je moet vooraf weten wat je tot stand wil brengen of bevorderen. Je bent voortdurend aandachtig bezig met gericht te observeren, wat ertoe leidt dat je een minder duidelijk zicht hebt op wat er zich voor de rest in de leefgroep afspeelt.

Als voornaamste meerwaarde, voor mij persoonlijk, vond ik dat het gebruik van contactreflecties steeds een houvast aan mijn handelen bood. De cliëntgerichte basishouding van echtheid, empathie en aanvaarding is hierbij zeker belangrijk.

Slotbeschouwing

Om af te ronden, zou ik graag deze methode willen opentrekken naar meer hulpverleningssituaties.

Ook voor personen die niet contactgestoord zijn, kan het gebruik van contactreflecties nuttig zijn. Personen zoals u en ik kunnen soms ook even het contact met de werkelijkheid, … kwijt zijn. Ik denk hierbij bijvoorbeeld aan crisissituaties, zoals personen die juist een auto-ongeval hebben meegemaakt. Personen moeten niet eerst het contact kwijt zijn, vooraleer contactreflecties toegepast mogen worden. Contactreflecties kunnen ook gebruikt worden om alleen contactbevorderend te werken. Hierbij denk ik bijvoorbeeld aan kinderen die schrik hebben omdat ze naar de tandarts moeten. Als de tandarts contactreflecties zou gebruiken, zou het kind misschien sneller voelen dat hij/zij begrepen wordt. Het contact kan verstevigd worden en het kind krijgt misschien minder schrik.

Person-centered work and Pre-Therapy contact work in residential psychiatric milieu treatment with elderly

In this article, attention is first directed to milieu therapy as it may be elaborated upon in working with people of old age in a psychiatric hospital. Based on the work of Marlis Pörtner (1998; 2008), this contribution centers on a description of how a person-centered approach might be applied within this milieu therapeutic framework. This description is guided by the familiar therapeutic relational core conditions from the client-centered approach. Finally, Pre-Therapy contact work (Prouty, Van Werde, & Pörtner, 2001; Sanders, 2007) and its application in this setting are considered.

Using contact work in interactions with adults with learning disabilities and autistic spectrum disorders

Some people with learning disabilities find it hard to interact with other people. This project looks at one way of making interaction easier. We tried a way of measuring whether this worked or not. The project was useful, but there is more work to be carried out on how to measure this.

This article describes a project about using contact work with people with learning disabilities and autistic spectrum disorder. People with learning disabilities and additional autistic spectrum disorder are at risk of becoming socially isolated because of their difficulties in interacting with others. Contact work is a form of Pre-Therapy, which is based on Carl Rogers’ person-centred approach. It focuses on psychological contact, which is the first of the ‘necessary and sufficient’ conditions for therapeutic relationships. The approach was developed by Gary Prouty and has been used with people who find it difficult to establish psychological contact because of their emotional, mental health or cognitive problems. The approach was used with two clients with learning disabilities and autistic spectrum disorder. The sessions were evaluated using a tool created for the project that was designed to measure different contact behaviours. The impact of the project on both clients and therapists is discussed. In addition, the measurement tool is examined, and areas for further study are identified. The project was felt to be useful on a subjective level. The measurement tool was useful in collecting quantitative data but requires more work to capture more qualitative aspects of the work.
